0004013: feature wish: define CAML_NAME_SPACE per default

Description

In bigger mixed C/Ocaml projects the identifiers in compatibility.h get in the way. For instance

#define flush caml_flush /*SP*/

will very likely cause trouble.

I suggest to document CAML_NAME_SPACE in Chapter 18 "Interfacing C .."

Further, I believe compatibility.h should be disabled per default.
